   Proposal Description:
   Specific Aim 1: To replicate the recent findings of O???Bryant et al (2008, in press) that a higher cut score on the MMSE is required for detecting dementia among highly educated individuals (i.e. those with 16 or more years of education).  Specific Aim 2: To evaluate the diagnostic accuracy of MMSE cut scores in detecting dementia among highly educated individuals from diverse racial/ethnic backgrounds.  Specific Aim 3: To evaluate the diagnostic accuracy of MMSE cut scores in highly educated Spanish-speaking individuals.     Primary Analysis Plan  We will follow the analytic plan outlined by O???Bryant et al (in press).  Specific Aim 1: A receiver operating characteristic curve will be generated comparing controls to those with dementia. Estimates of sensitivity and specificity of MMSE cut scores from 30-16 will be calculated. Next, positive (PPV) and negative (NPV) predictive values for each cut score for a range of base rates will be provided. Next, the same process will be followed comparing controls to those with MCI and again comparing controls to those with cognitive impairment (MCI and dementia).  Specific Aim 2: The above-mentioned analyses will be repeated for those who self-identify their race as African American and again for those who self-identify their ethnicity as Hispanic. For both of these, non-Hispanic whites will be used for comparison purposes.  Specific Aim 3: The above-mentioned analyses will be repeated comparing Spanish-speakers to English-speakers
